The state is out of free kits. Upper Missouri District Health Unit gives free kits in Williston, Watford City, Stanley, and Crosby while supplies last. Everyone else: order by mail, $18 from the Lung Association or $20 from Kansas State University.

Do not use the old free-kit form.
